Depends on the FOMEMA report date. If the health report is more than 6 months old, a new medical check-up is required.

What if the work permit is near expiry?

Work permit must be valid for minimum 3 months from application date. If near expiry, the work permit must be renewed first before proceeding with green card renewal.

Does the employer need to be involved in this process?

Yes, employer must provide endorsement letter and ensure the company has valid CIDB registration.

Can I apply if my passport has been replaced?

Yes, but you need to provide the old passport as reference. Immigration department needs to verify work permit transfer to new passport first.
